To remain active, all workers must review & watch the following 3 items annually before completing the form below.

1. REVIEW the Statement of Faith Determine if you are in agreement to serve with CEF.
2. REVIEW the Ministry Compliance Agreement Determine if you are in agreement to serve with CEF.
3. WATCH the "Protecting Today's Child" Video Understand your responsibility regarding the children and CEF.
